Practice Improvement Tools
These practice improvement tools are resources for assessing current clinical practice, developing and implementing a plan for practice change, and measuring the results.
This is a set of measures that healthcare professionals may use to evaluate their practice and track their performance improvement progress as related to counseling parents/guardians of a child about secondhand smoke exposure.
This is a questionnaire for physicians and other clinicians that may be used to evaluate their knowledge, attitude and clinical practice as related to secondhand smoke.
This is a questionnaire for a parent, grandparent or guardian of a child that may be used to evaluate child exposure to secondhand smoke and respondent knowledge and behavior as related to secondhand smoke. Available in English and Spanish.
The Care Model (CM) is a unique and proven approach for implementing proactive strategies that are responsive to both patient and provider needs. Developed by Improving Chronic Illness Care (ICIC), a national program of the Robert Wood Johnson Foundation, the model integrates community, organizational, practitioner, and patient systems. Based on published results, the Care Model promotes “continuous healing relationships” characterized by planned sets of interactions and interventions over time to optimize quality and delivery of more efficient and effective health care.
